edit shipping address form page doesn't show

Hi,
i have a problem with the edit shipping address form. When i try to buy a product and i insert my billing address it' all ok, but if i try to edit add/edit my shipping address (click on add/edit shippin address button) i can't see the form, i see only the save or cancel button, without the form field to inser the shipping address.
Can you help me please?

In vm admin unde Configuration->Shopper Filed see it you have them and are fields enabled.
