HOT Survey module

Knowing your customers opinion is very important.
I was planning to use some survey (a popup and form) with my template.
If you had a survey extension to be inlcuded,would be great.

While you don't have this module, can you suggest any 3rd part extension to use with Hot Software Template?

We used it with Hot Betting template and it works fine.
