Shopping cart checkout form field issue

Ivan or anyone else with an answer,
I have attached a screenshot of the shopping cart checkout Step 1 of 4. As you can see from the screenshot, the customer form fields for billing and shipping have no fields for adding the required information. I am not able to fill in any information for company, name, address, phone or fax and the country is showing on the screen as United States and the email shows info@hotjoomlatemplates.com

I have looked all through the template and the Virtuemart store settings and can't seem to find a form field edit function. Any help is appreciated. The site is currently offline so let me know if you need access to the backend to look into this.

That links (Update Address) and (Shipping Address ) are not working???
