shipping rate schedule config question

I ship based on price not weight.. how do I configure the shipping module with my rate schedule? It shows lowest weight and highest weight - I want to create $3.00 - $5.99 = $3.89 shipping for example

ALSO - how do I add additional lines for the rates e.g., up to $6.00 - $9.00 = $7.98 shipping

However:

if I need to do this by weight, how do I add multiple rates e.g., up to 13 oz = $3.89
14 oz to 2 lbs = $7.89, etc...

thank you for the link - unfortunately there is no real answer for the problem I have - I SUCCESSFULLY created my rates for my standard shipping - however the shipping does not show in the cart or checkout.. is there any suggestion you can provide? While the cart does weem to work in general - I'm hoping the install I payed for would cover making sure I can open my store.. can you take a look? I ONLY have standard shipping checked, and my rates are saved. cant figure out why this is not showing up..
