Adding a Top Menu Item

Hi, I can't seem to add a new menu item to the Top Menu in the correct manner.

The formatting/style/placing is completely wrong.

Please give me some advice, I want to make my own link.

there is something very odd with what I see in your picture it seams you have two menus in user3 position, one normal and other without styling. Did you set:
Menu Tag ID = nav if menu module settings.
