Add/Edit Shipping Address was created by chicago

Hi,

When clicking 'Add / Edit Shipment Address' the appropriate page is displayed (Add / Edit Shipment Address) but no fields are shown.

Cn you have a look please

In virtuemart admin there is option where you manage user fields, check that maybe they are all unpublished
