Your link is not working, but I guess your question is related to the "Contact Us" form on the home page. This form comes from the simple Rapid Contact module. You can change some module parameters, but you can't add or remove fields without changes in the module code. The module file is /modules/mod_rapid_contact/mod_rapid_contact.php

If you need a more complex form, please install a premium form extension of your choice from extensions.joomla.org/category/contacts-...dback/contact-forms/ or extensions.joomla.org/category/contacts-and-feedback/forms/
