Change Field in Mod Rapid Contact

Hi.
I want to remove the Date field in the Mod_rapid_contact and add a Name field.
What steps should I take?

I tried to tweak the mod_rapid_conta.php file but... it but did not result because i don't know how to do it.
Any suggestion is welcome
