Module link to menu item for booking calendar

Hi

I uploded a category extension called PBBooking. I want to be able to link this category to the module i renamed Rates/Schedule. I was able to get the booking form to work from the menu (menu item type PB Booking)

but i cant get it to work from the module. Please can you help. I have attached a file with screen shots

We can only check if there is some script conflict with out template, send link

I got this issue resolved however there seems to be an issue with language settings.
This booking extension displays a calendar however it also displays some form of "coding"
e.g. pbbooking_com.....so when i am on the booking form it does not display the fields correclty but rather some form of code which represent those fields

Why do you think it comes from template, send link so we could see

I dont have a link at the moment as I need to find a free host provider but u
